Observation

One of three sampled child files indicated that the child has an IEP but is missing Individual Care Plan (ICP) and missing information regarding child's health history on enrollment form. Another child has allergy and Epi-pen. Missing verification that staff caring for the children with health conditions/special needs are trained.
